Do you believe in the Lord Jesus Christ as you have been taught in the Bible and the Book of Mormon? (Acts 16:31, Mark 16:16, John 11:25-26, 2 Nephi 9:23, Mosiah 3:12) • The scriptures make it clear that salvation comes only through faith in the Lord Jesus Christ.
Have you forsaken and repented of all sin as explained in the scriptures? (Ezekiel 18:30-31, Isaiah 1:16, 2 Corinthians 7:10, Alma 34:33, 3 Nephi 9:22) • All of us have sinned in a greater or lesser degree and must repent in order to progress toward salvation. A person repents when he or she turns away from evil and obeys God’s commandments. All of us are required to repent of all our sins before entering before God's Kingdom.
Were you baptized by immersion, following Christ's example? (John 3:5, Mark 16:16, Acts 2:38, Colossians 2:12, 3 Nephi 11:25-26, Mosiah 18:12-16, 2 Nephi 31:13-18) • Instead of baptism by immersion for the remittance of sin, as taught by Christ's example, many churches practice baptism by pouring or sprinkling. Some do not baptize at all.
Were you baptized by one holding the proper priesthood authority to perform baptisms? (Matthew 28:16-20, John 15:16, Hebrews 5:4, Mark 11:28, 3 Nephi 12:1-2, 3 Nephi 11:18-22)
Have you received the Gift of the Holy Ghost by the laying on of hands? (Acts 8: 14-17, Acts 12:14-17, Acts 19:2-6, 3 Nephi 18:36-37, Moroni 2:2, 2 Nephi 31:17-18)
Have you been a loving and faithful husband or wife and father or mother as instructed in the Bible and the Book of Mormon?
Have you loved and served your fellow man as required in the scriptures?
Have you cared for the poor and the needy as instructed in the scriptures?
Have you lived the Law of Tithing as taught in the scriptures??
Did you endure and remain faithful to the end of your life as taught in the scriptures?
